Humana's Primary Care Organization is seeking a full-time Float Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at our clinic in Orlando, FLAs a Nurse Practitioner (NP) or a Physician Assistant (PA) at CenterWell Senior Primary Care. You will be part of a collaborative, diverse team. This team includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.

You will report to the Associate Medical Director and float as needed throughout our Orlando, FL market.

Your Role & Responsibilities
- You will provide comprehensive care to seniors following clinical standards.
- You will collaborate daily with your care team to ensure coordinated, high-quality care.
- You will manage referrals, hospital/SNF coordination, DME, and home health services.
- You will document care with support from quality-based coders.
- You will participate in on-call rotation and support growth plans at your center.
- You will have experience with providing value-based care.
- You will work on-site.
What We're Looking For
Required:
- Master's degree in nursing with board certification, or completion of a PA program with board certification.
- Active, unrestricted NP or PA license in the state of Florida.
- At least one year of experience in value-based care, primary care, or Medicare Advantage settings.
- Active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain them.
- Familiarity with HEDIS quality indicators and Medicare guidelines.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
