About this job
About Hike Medical
Hike Medical is building the defining company in musculoskeletal care. We sit at the intersection of AI, robotics, and healthcare, operating across three product lines: a proprietary AI-vision platform that turns a 30-second, web-based foot scan into custom 3D-printed orthotics; an AI agent platform that automates the entire DME workflow from pre-visit processing through claims and revenue cycle; and SoleForge, our vertically integrated 3D-printing factory producing custom medical devices at a scale the industry has never seen.
Insoles today, full DME tomorrow, bionics by 2040. Custom insoles are just the wedge. Our long-term vision is bionics: AI-designed, robotically manufactured orthotic and prosthetic devices at scale, replacing a fragmented, manual industry that hasn’t changed in decades. Read the full vision at bionics2040.com.
We've stealthily raised $22M through Seed and Series A, backed by top-tier investors who backed companies like OpenAI, Anduril, and Mercury early. We run a fast, results-first, high-ownership culture out of our new SF Rincon Hill office. If you want to work at the frontier of AI, manufacturing, and healthcare, this is the place.
The Role
As a Staff Software Engineer, you will set the technical direction for Hike’s platform across our clinical software, AI agents, and manufacturing. You will own the hardest problems that cut across products, make architecture decisions that hold up as we scale, and raise the bar for every engineer around you. This is a builder role first: you will still write a lot of code.
Why this is career-defining
You will shape the technical foundation of a company redefining musculoskeletal care.
Your architecture decisions will set what Hike builds on for years.
You will have the scope of an engineering leader with the hands-on impact of a builder.
Your code will turn into real devices that help people move without pain.
What you will work on
Design the core platform that connects Hike’s AI, clinical, and manufacturing systems into one system of record.
Lead the company’s most complex technical initiatives, from enterprise integrations to healthcare data infrastructure.
Set the standards for reliability, security, and compliance across engineering.
Mentor engineers and partner with product and design to shape what we build next.
What we look for
8+ years building production systems, with a track record of leading large technical efforts end to end.
Our core platform is TypeScript and Node.js, with NestJS APIs and PostgreSQL through Prisma, running on AWS.
Deep experience in backend architecture, distributed systems, and data modeling.
Fluency with modern cloud and tooling. Experience putting AI into production is a plus, not a requirement.
Product-minded: you think about how your work affects real users.
A strong communicator who challenges assumptions and collaborates across engineering, AI, and product.
Mission-focused: you are not here to optimize ad clicks, you want to build technology that changes lives.
What we ask
In person in our San Francisco Rincon Hill office, 5 days a week.
This is a hard job with a high bar. In return the work is unusually rewarding, and you will see your impact quickly.
High ownership and low bureaucracy. Results come first.
